2012 - 2013 Applications

It is easy to apply. Take a moment to download and fill out one of the below applications. Return it to Resurrection School with:

• A $50 application fee (cash or check made payable to Resurrection School)
• A copy of your child's baptismal certificate
• If not baptized, a copy of your child's birth certificate
• A letter of recommendation from a teacher for children entering grades 1 - 8
• A copy of your child's last report card for children entering grades 1 - 8
• A copy of your child's latest test scores for children entering grades 3 - 8
• The parent questionairre
